Welcome Form – needs to be completed if you have not visited our office since October 2019
COVID-19 Consent Form – needs to be completed before every office visit

November 24, 2020

To all our patients,

As the COVID 19 cases rise in our state, we want to reach out to let you know we are continuing to monitor the current health situation.  We will continue our vigilance to keep our patients and employees safe.  With that in mind, our procedure for checking in for appointments will change.

New procedure:

With the cold weather upon us, we will be asking you to call from your car when you arrive for your appointment, whether it is for an exam or with optical.  When we are ready for you, we will call you back.  We ask for your patience as we clean and keep a limited number of people in the building to allow for social distancing.

Please note we now request that you make an appointment for eyeglass adjustments and repairs and will continue to do this outside.  There is a drop box on the front porch for picking up glasses, contact lenses and any other optical needs.

Our phone number to notify us that you are here is (802) 388-2811.  You will be prompted to press (*) to indicate you are here.

Continue to be vigilant:

For everyone’s health and safety, it is paramount that you arrive wearing a well fitting mask that consistently covers both the nose and mouth even when speaking.  Masks must be worn the entire time you are in our office.  Please do not bring any food or drink into our office.

Please be alert to any sign of illness you may be experiencing that may be linked to COVID 19.  We continue to take temperatures and ask you to cancel ahead of time if you have symptoms of a COVID infection.  Per the Vermont Department of Health, we cannot allow anyone with symptoms to enter our office.  As well, we still cannot allow patients to enter our facility who have travel outside of Vermont or have entertained out-of-state visitors.

Your appointment:

Please know that you will be receiving the same exceptional eye care at our office as in the past.  Your appointment time slot has expanded due to our commitment to social distancing.  Please take full advantage of your time with us.  Please plan on being dilated and selecting glasses if you so choose during that time.

We are looking forward to continuing to provide you with all your eyecare needs.  We thank you for your patience as we navigate this difficult year.

Stay well and be safe,

Drs. Dan, Diane, Monica, Kayla, Emily and Kendall, Tracy, Chris, Miranda, Jess, Heather, Ed, Ally, Cindy and Jeanne
